This page contains key terms relating to Catalog of Care Reports. Please note that these definitions reflect the current manner in which these terms are used in the Subcontracted Services Subentity of the Contract Data Set Pamphlet 155-2, which in some instances may differ from their historical meaning, or use in other contexts.

Amendment Date

This term refers to the date when the Subcontract between the Provider and the Managing Entity was amended.

Amendment Number

This term refers to a number indicating the amendment to the Subcontract between the Provider and the Managing Entity.

Contracted Amount

This term indicates the total dollar amount subcontracted to the Provider by the Managing Entity per discreet Covered Service or Project under the initial or amended Subcontract between the Managing Entity and the Provider.

Contract Number

This term refers to a number that uniquely identifies the Contract between the Managing Entity and the Department of Children and Families (DCF).

Covered Service

This term refers to covered service, as defined in 65E-14.021, F.A.C., to which the contracted amount is used to purchase and provide services to individuals under the initial or amended Subcontract between the Provider and the Managing Entity.

Effective Date

This term indicates the date when the initial or the amended Subcontract between the Provider and the Managing Entity became effective during the fiscal year for the contracted amount.

Expiration Date

This term indicates the date when the initial or the amended Subcontract between the Provider and the Managing Entity will expire during the fiscal year for the contracted amount.

Payment Method

This term indicates the method used for payment to the service Provider by the Managing Entity for each discreet Covered Service or Project under the initial or the amended Subcontract between the Provider and the Managing Entity. Methods of payment, currently used for reporting Contracted Services data into FASAMS, include the following: Capitation Rate, Case Rate, Cost Reimbursement, and Fee for Service.

Payment Rate per Unit of Measure

This term indicates the per unit dollar amount negotiated between the Provider and the Managing Entity for each Covered Service or Project under the initial or amended Subcontract.

Payment Type

This term indicates whether the Covered Service, under the initial or amended Subcontract between the Provider and the Managing Entity, should be reported and paid on the basis of utilization or availability, as specified in 65E-14.021, F.A.C. The types of payment for Projects are not predefined; they are reported by the Managing Entity when Contracted Services data are submitted into FASAMS.

Program Area

This term indicates the program area in which the contracted amount is used to purchase and provide discreet or bundled services to individuals under the initial Managing Entity Subcontract or Subcontract amendment. The following are the program areas currently used to collect and report Subcontracted Services data in FASAMS:

Adult Mental Health – Include adults with mental health disorders aged 18 years and above

Children Mental Health – Include children with mental health disorders aged 17 years and below

Adult Substance Abuse – Include adults with substance abuse problems aged 18 years and above

Children Substance Abuse – Include children with substance abuse problems aged 17 years and below

Project

This term indicates the project, as defined in Pamphlet 155-2, Table 8 of Appendix 1, to which the contracted amount is used to purchase and provide bundled services to individuals under the initial Managing Entity Subcontract or Subcontract amendment.

Provider Name

This term refers to the name of the service Provider that has a Subcontract with the Managing Entity.

Purchased Capacity

This term refers to total service units contracted to the service Provider by the Managing Entity per discreet Covered Service or per Project under the initial Managing Entity Subcontract or Subcontract. This total is calculated by dividing the Contracted Amount (Numerator) by the Payment Rate per Unit of Measure (Denominator).

Subcontract Number

This term refers to a number that uniquely identifies the Subcontract between the service Provider and the Managing Entity.

Submitting Entity Name

This term refers to the name of the Managing Entity that is under Contract with the Department of Children and Families and is submitting the Subcontracted Services data into FASAMS on behalf of the network service Providers.

Unit of Measure

This term refers to unit of measure used for each discreet Covered Service or Project under the initial or amended Subcontract between the Provider and the Managing Entity. Units of Measure, currently used for reporting Contracted Services data into FASAMS, include the following: Day, Direct Staff Hour, Dollar, Dosage, Non-Direct Staff Hour, Monthly Service, Quarterly Service, Weekly Service, and Other.
