The Office of Child Care Regulation regulates different types of child care providers and can only investigate complaint allegations relevant to the governing statutes and/or rules for child care services. Below are examples the types of health and safety complaint allegations we can investigate:

Staff-to-child Ratio, Inadequate Supervision, Training Requirements, Background Screening, Nutrition, Food Safety, Record Keeping, Physical Environment, Transportation, Illegal child care operations, Accident/Incident Reporting, etc.

DO NOT USE THIS FORM TO REPORT ANY ABUSE, NEGLECT OR EXPLOITATION OF ANY CHILD OR VULNERABLE ADULT. Florida law requires anyone with information to report abuse directly to the Florida Abuse Hotline at 1-800-962-2873 or by using our online reporting tool .

In the Complaint Narrative section please include as much detailed information as possible. This includes a description of the incident as well as answers to the basics questions like who was involved, what occurred or was observed, when did the incident happen, where did the incident occur, etc.

You may submit your complaint anonymously, however if there is not enough information entered into the complaint narrative, and we cannot contact the complainant for more information, the complaint may not be investigated.
