Submit a Single Audit Report
Department contracts require the submission of either a complete Financial Reporting Package or a certification that no audit was required within 180 calendar days of the end of a provider’s fiscal year. Financial Reporting Packages or the certifications are to be submitted electronically to the Single Audit Unit at: HQW.IG.Single.Audit@myflfamilies.com.
If a provider does not meet the threshold for a federal and/or state single audit, the Department requires submission of a certification that no audit was required. Certifications should meet the following requirements:
- On company letterhead;
- Contain a statement that the provider did not expend more than $750,000 (current federal threshold) in federal awards for the fiscal year (fiscal year should be identified);
- Contain a statement that the provider did not expend more than $750,000 (current State of Florida threshold) in state financial assistance for the fiscal year (fiscal year should be identified); and
- Signed by a board member or officer of the company.
Federal Audit Clearinghouse
The Federal Audit Clearinghouse website contains Single Audit reporting packages for most nonprofit providers; Indian Tribes and Tribal Organizations may opt not to authorize the Federal Audit Clearinghouse to make the reporting packages publicly available. Reports prior to fiscal years ending on December 26, 2014 will have a summary form available. The summary form contains information related to funds awards and any reported findings by the CPA firm. For fiscal years beginning after December 26, 2014 the form and the actual Financial Reporting Package are available on the website.
